The Application Process
Before submitting an application for designation, applicants should read
the Exchange Visitor Program regulations to determine if they meet the
eligibility and general program requirements. The nonrefundable application
fee, payable to the United States Department of State, is $799.
The Checklist for Preparing Form DS-3036, Exchange Visitor Program Application
provides detailed instructions for completing an application for designation.

An application for designation includes three parts: A completed, signed,
and notarized Form DS-3036; a written proposal defining the program parameters
and explaining how the sponsor will maintain compliance with program
regulations; and, supporting documents. The written proposal responds
to both general administrative requirements and to the specific program
requirements for the category or categories for which designation is sought.
The Successful Proposal
The successful application proposal includes, but is not limited to:
- Proof that the eligibility requirements are met
- Specific information about the staff, its size,and its hierarchical
organization
- A description of the screening and selection process, responding to
both general and specific program category requirements. Copies of applications,
reference letter forms, and other paperwork to be completed by the applicant
participant should be submitted.
- A line-item breakout of all fees and program costs participants can
expect to incur. If program requires the use of host families, a line-item
breakout of all fee and program costs they can expect to incur should
be provided as well.
- A copy of the medical insurance policy, with a schedule of benefits,
to be offered to program participants. If the applicant sponsor does
not intend to offer insurance coverage, a clear description of how compliance
to program regulations will be maintained should be included.
- A summary description of the orientation to be provided to participants
that responds to both general and specific program category requirements.
Sample copies of materials to be given to program participants as pre-arrival
and arrival orientation materials should be submitted to the Department
in support of the application.
- Applicant sponsors should outline their strategy for monitoring their
participants, for providing contact information and for notifying the
Department in the event of a serious problem, as required by the program
regulations. Please refer to specific program category regulations as
well as to general administrative requirements for details.
- Any specialized program information, such as training plans, specified
in the category regulations.
- A brief statement on how blank DS-2019 forms will be stored.
Supplementary Documentation
Supplementary Documentation all applications includes:
- A true copy of the Articles of Incorporation or State Charter
- A copy of the By-laws of the organization
- A Certificate of Good Standing, available from the State Department
of Commerce or the State Secretary of State
- Copies of all required local, state, federal licenses and evidence
of accreditation
- A Financial Audit
- Notarized Citizenship
Certification forms for all responsible and alternate responsible
officers
Additional Documentation for Trainee Program
Applicants
Applicants for designation of a Trainee program must also submit the following
documentation:
Timing
Initial review of a new application may require up to four months, and
is completed by the Department officer responsible for the oversight of
the category or categories for which designation is sought.
